Emily Kopek was the only person that actually took a real interest in trying to help me get well. My other doctors never took the time to help me figure out what was going on - they just told me to eat right and exercise and I'd be fine, but I had undrrlying issues they just missed. I think without her assistance, I would be in a critical situation at this point. She sent me to genetic testing and we found out I had a then newly-discovered auto-immune issue dealing with MTHFR. Emily also did extensive testing that diagnosed me with both Epstein-Barr and Lyme disease. Emily helped balance my hormones and avoid an unnecessary hysterectomy. Does your office accept insurance?
We do not accept insurance for office visits. Most bloodwork labs through LabCorp or Quest in our office can be billed to insurance. We do not currently offer any payment plans. We do not accept Care Credit. We happily accept HSA and FSA cards as well as all major credit cards, checks, and cash as forms of payment.
Most specialty tests outside of bloodwork are self-pay, with a few exceptions that can bill insurance. If any specialty testing is recommended, be sure to verify whether they are billable to insurance or are self-pay only. Some testing companies we work with offer payment plans.

Restorative Functional Medicine will likely want bloodwork completed as a part of my diagnosis and treatment.
LabCorp is in the office of Restorative Functional Medicine as a courtesy and is in no way affiliated with the business of Restorative Functional Medicine.
Restorative Functional Medicine have no way of knowing what insurance will cover for the bloodwork, how much will be covered, or there will be out-of-pocket expenses for those labs.
If there is a bill for bloodwork, the patient will need to contact their insurance and/or LabCorp or the labs were completed.

Patients understand that it is their responsibility to determine what bloodwork insurance will cover and if they have to meet a deductible or will incur any out-of-pocket expenses.
How do I schedule an appointment? What is the process like?

Confirmation Required: You will receive a text and email confirmation upon scheduling. If you do not receive these, your appointment is not booked. Please contact us to check.

New Patients & Paperwork

Forms: Complete your new patient paperwork in your portal’s “Questionnaires” section at least one week before your visit to secure your spot.

Payment Info: A credit card on file is required. Please reach out to our office to supply the card information.

Billing: Appointments are billed 48 hours (two business days) in advance.
Appointments & Labs

Initial Visit (60 mins): We will review your medical history and discuss diagnostic testing options.

Follow-Up Visit (30–45 mins): Scheduled 4–6 weeks later once all lab results (bloodwork, cortisol, stool, etc.) are in.

Lab Results: To avoid confusion, your results will be shared in your patient portal after you have reviewed them with your provider during the follow-up visit.

Ongoing Care: Subsequent follow-ups are typically scheduled quarterly, then every 6 months, based on your provider's recommendations.
Do you write prescriptions?
Supplements

Ordering: Providers recommend supplements via an online professional pharmacy rather than keeping stock in the office.

Discounts & Protocols: You can access prebuilt protocols and our practitioner discount by creating an account through our Fullscript Dispensary Link. You do not need a prescription or patient status to order.

Prescriptions & Refills

Availability: Prescriptions are processed Monday through Thursday. The office is closed Friday through Sunday.

Refill Timeline: Please allow 48 hours (two business days).
Do you offer virtual or telehealth visits?
We do offer telehealth appointments as a convenience for patients that do not live local to our office, for certain sick visits, or for the occasional follow up visit.

If you live within three hours of our office, you must be seen in-person at least once per calendar year.
What is your cancellation policy?
Due to short notice cancellations or patients not showing up for scheduled appointments, we will bill for your appointments 48 hours, or two business days, prior to your appointment. We understand things happen. If you need to cancel your appointment, we must have at least 48 hours, or 2 business days’ notice of the cancellation, or notification by Thursday for any Monday appointment cancellations, in order to not be billed for your appointment.

If you are not feeling well the day of your appointment, we can change your appointment to a telehealth visit for that day to avoid the short notice cancellation
